Templates for SOAP, Upper Extremity, Lower Extremity, Cervical, Lumbar, Neuro/Vestibular, Pelvic Floor, and TMJ are included and customizable.  Each template includes an Evaluation, treatment note/progress note/discharge, Outcome Measures, and superbill. Information is brought forward from the Eval to the Rx note,PN,DC sheet. The header will show your company information and logo on each page.  All templates add your total CPT code times for you.
Each template comes with a ‘Unsub Menu’ that allows you to load a side window on the same screen and update your shortcuts or even perform a patient intake on the same screen. 
 
There is a navigation bar in the first row to easily jump from tab to tab or section to section. You can select to have email reminders per patient as well as which email reminders to receive. You even have the option to click on a button and save as a PDF directly to the patient folder.
 
ALL TEMPLATES ARE MADE FROM GOOGLE SHEETS. That means you can adjust them to fit your needs without needing to contact Unsubscribed Healthcare. If something doesn't fit simply change the cell to what you would like. 
 
Don't see a template for you? Do you want something completely different? We got you covered with a folder for you to try and make your own. Make a copy of the template inside that folder, name it what you would like. In that new copy leave the Info, and Database tabs alone. Keep everything above Row 24 in the EVAL SHEET, Superbill, and Rx Note,PN,DC sheets the same and you should be able to create your own templates and have the script I wrote continue to work! Move it to the designated folder once you have tested it and then it will copy to each new patients folder that is automatically created. Let me know if you make something truly amazing and we can show it off to everyone! Note: DONT CHANGE THE NAMES OF THE TABS. THIS BREAKS THE CODING. 
 
Don't see the Outcome Measures that you want? You can add to the list that has been created and you can have it calculate the totals with a little knowledge of how to write formulas.
 
New patient folders are automatically created when a patient intake form or a patient subjective/consent to treat form is completed. You can select the format of the Folder created between First Name Last Name, First Name Last Name Patient ID, and Last Name First Name. You can also select the destination of the new patient folders. That way you can keep patient folders based on the year or however you choose to organize. A copy of the templates mentioned above is added to the new patient folder with the patient's name when it is created. The templates will populate the submitted subjective information to the templates when the patients name and ID has been selected. 
 
The PT ELITE PACKAGE includes the Data Center. The Data Center has sheets that help with Billing, Clearing House, Patient Intake, Authorizations, Visit Tracking, all shortcuts forms, and the ability to create CMS 1500 forms with and without the grid. All shortcut information is updated to every template created when connected to the Data Center. This helps to improve your documentation time.

You are able to create CSV files for submission to a clearing house (this is still being tested). Save as a PDF directly to the patient drive folder and not to your computer. I do not charge per clinician in your business.
 
Create your own Home Exercise Program! The HEP file takes the shortcuts from your flow sheet shortcuts and adds it as options to your HEP. Add your own instructions and pictures to start creating your very own HEP. 

***You will need to get a paid version of Google Workspace and sign a BAA with Google in order to be HIPAA compliant. If you do not sign a BAA with Google, Google workspace is not HIPAA compliant. 
 
Certain levels of Google workspace allow you to also make a schedule in which patients can schedule themselves. 
 
If you have a paid version of Google Voice, you will be able to answer from the side Tab on every Template.
